Job summary

Transitions Hospice is seeking an experienced RN to provide compassionate, patient-centered hospice care in Champaign, Piatt and surrounding counties. The position offers a competitive salary of $70,000 to $90,000 a year and a schedule of 8 AM–8 PM with alternating 4-day and 3-day weeks, including weekends in rotation with on-call coverage.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
• Building and adjusting personalized care plans in collaboration with physicians and care teams
• Administering medications and treatments per physician orders
• Keeping clinical documentation accurate, timely, and meaningful
• Serving as the communication bridge between families, physicians, and community health partners
• Educating patients and caregivers so they feel confident and supported
• Participating in on‑call rotation per team policy
