Job summary

Transitions Hospice is seeking a compassionate RN to cover overnight shifts in our Illinois coverage area. As a Night Nurse, you will assess, treat, and communicate patient needs with clinical confidence, delivering high-quality care during 8pm–8am on an as-needed basis.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or related setting preferred.

Responsibilities

• Delivering professional nursing care across the full nursing process within your scope of practice.
• Conducting thorough patient and family assessments and contributing to individualized care plans.
• Completing all visit types as needed to meet overnight patient and family needs.
• Administering medications and treatments per physician orders.
• Identifying nursing diagnoses from health assessment data and responding with sound clinical judgment.
• Recording accurate, timely clinical notes including pain and symptom management outcomes.
• Communicating changes in patient condition to physicians and obtaining orders as required.
• Coordinating with community health partners to support continuity of care.
• Ensuring equipment and necessary services are in place for patients.
• Maintaining and growing your clinical skills through continuing education and in-service participation.

Coverage Territory: Will, Kendall, DuPage, Kane, Dekalb, Lee, Ogle and Cook and surrounding counties

Schedule: 8pm to 8am on an as needed basis to fulfill patient care needs.

Pay Range: $40.00 - $45.00 an hour

What You Bring

• Active RN license in the state of employment
• Graduate of an accredited school of Nursing.
• Clinical experience in hospice, home health, or a related setting preferred
• Comfort and composure working independently during overnight hours
• Strong assessment skills and sound clinical judgement in high-pressure moments
• A calm, reassuring presence for patients and families during nighttime distress
• Excellent documentation habits and communication skills
• Valid driver's license and willingness to travel locally
