Job summary

Transitions Hospice - Illinois is seeking an RN Case Manager to coordinate care for patients in the Mattoon/Charleston area. You will lead the interdisciplinary team, conduct assessments, build care plans, and ensure documentation is accurate and timely.

The role requires an active RN license in the state, 1+ year hospice or home health experience preferred, and excellent communication. Competitive salary with benefits and rotating on-call coverage are provided.

Responsibilities

• Coordinate care and guide the interdisciplinary team.

Coverage Territory: Mattoon / Charleston and surrounding areas

Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation

Salary Range: $70,000 - $85,000 a year

What You'll Do

As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:

• Educating patients and caregivers so they feel confident and supported
• Participating in on-call rotation per team policy
