Job summary

Transitions Hospice in Naperville, IL, is seeking an RN Case Manager to serve as the clinical anchor for patients and families, coordinating care with the interdisciplinary team.

You will assess patient needs, develop care plans with physicians, administer treatments, ensure accurate documentation, and participate in on-call rotations. The role follows an 8 AM-8 PM schedule with alternating 4-day and 3-day weeks, including weekends.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited school of Nursing.
• One (1) + year Hospice, home health or related organization experience preferred.
• Compassion for patients and families navigating end-of-life care.
• Strong clinical judgment and the ability to work independently in the field.
• Excellent communication skills across care teams and families.

Responsibilities

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs.
• Building and adjusting personalized care plans in collaboration with physicians and care teams.
• Administering medications and treatments per physician orders.
• Keeping clinical documentation accurate, timely, and meaningful.
• Serving as the communication bridge between families, physicians, and community health partners.
• Educating patients and caregivers so they feel confident and supported.
• Participating in on-call rotation per team policy.
