Job summary

Pride Health is seeking a Registered Nurse (RN) - Home Health to support our client's healthcare facility in Lincoln, NE. This is a full-time local contract position with a schedule of 9:00 AM to 5:00 PM, Monday to Friday. Location is Lincoln, NE 68510 with local travel required.

Contract length is 14 weeks with a pay range of $48.00 to $51.00 per hour. Mileage reimbursement is $0.50 per mile. Responsibilities include SOC, ROC, and discharge visits, care planning, and coordination with the care

Qualifications

• Active RN license in the state.
• Minimum 2 years of Home Health experience.
• OASIS experience required.
• Homecare Homebase (HCHB) experience required.

Responsibilities

• Perform comprehensive patient assessments (SOC, SOC, ROC, discharge visits).
• Develop and evaluate Plans of Care with physicians and team.
• Provide skilled nursing care including wound care, venipuncture, IV care, catheter management, meds, education.
• Maintain accurate clinical documentation per Medicare/payer/agency requirements (HCHB).
• Coordinate care with physicians, therapists, caregivers, and home health aides.
• Participate in on-call rotations and interdisciplinary meetings.

Job Details:

Schedule: Full-Time | 8 Hours Per Day | Monday to Friday | 09:00 AM to 5:00 PM

Float / Coverage Area: Local Travel required

*Pay offered is based on experience, expertise, credentialing, and education.

Duties:

• Complete comprehensive patient assessments, including Start of Care (SOC), Recertifications, Resumption of Care (ROC), and Discharge visits.
• Develop, implement, and evaluate individualized Plans of Care (POC) in collaboration with physicians and interdisciplinary team members.
• Provide skilled nursing services including wound care, venipuncture, IV care, catheter management, medication administration, disease management, and patient education.
• Maintain accurate clinical documentation utilizing Homecare Homebase (HCHB) and ensure compliance with Medicare, payer, and agency requirements.
• Coordinate care with physicians, therapists, caregivers, and home health aides to ensure continuity and quality of care.
• Participate in on-call rotations and interdisciplinary team meetings as required.

Requirements:

• Active State Registered Nurse (RN) License.
• Minimum 2+ years of Home Health experience.
• OASIS Experience Required.
• Homecare Homebase (HCHB) Experience Required.
• Current CPR Certification.
• Valid Driver's License, Reliable Transportation, and Auto Insurance.
• 1-2 years of RN experience in a clinical care setting.
