Job summary

Pride Health is seeking a Registered Nurse - Home Health to support our client’s healthcare facility in Berwick, PA. The position runs Monday-Friday, 8:00 AM-4:30 PM, with a 14+ week contract and mileage reimbursement.

Responsibilities include skilled home visits, care plan development, patient education, and thorough documentation. Requires active RN license and BLS, with 1+ year Home Health experience preferred.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current BLS Certification.
• 1+ years of Home Health experience; HCHB experience a plus.

Responsibilities

• Provide skilled nursing care to patients in the home setting, developing and implementing comprehensive care plans.
• Educate patients and caregivers on disease management, medications, infection prevention, equipment use and self-care techniques.
• Maintain accurate clinical documentation and communicate changes in patient condition to the health care team.

Pride Health is hiring a Registered Nurse - Home Health to support our client's healthcare facility in Berwick, PA.

Job Details:

Schedule: Monday-Friday | 8:00 AM-4:30 PM

Location: Berwick, PA

Coverage Area: Columbia County, parts of Luzerne County, parts of Montour County, parts of Northumberland County, parts of Sullivan County, parts of Schuylkill County

Contract Length: 14+ Weeks (Possibility to Extend)

Pay Range: $60.00 - $63.00/hour (W-2) Plus Mileage reimbursement

Mileage Reimbursement Rate: $0.53/ mile

*Pay offered is based on experience, expertise, credentialing, and education.

• Provide skilled nursing care to patients in the home setting, performing comprehensive assessments, developing individualized care plans, and monitoring patient progress in accordance with physician orders.
• Educate patients and caregivers on disease management, medication administration, infection prevention, medical equipment uses, and self-care techniques to improve health outcomes and promote independence.
• Maintain accurate clinical documentation, communicate changes in patient condition to the interdisciplinary healthcare team, and ensure compliance with agency policies, Medicare guidelines, and professional nursing standards.

Requirements:

• License(s): Current and unrestricted RN licensure in state of practice
• Certification(s): Current BLS Certification
• Years of experience: 1+ years of Home Health experience and Homecare Homebase (HCHB) is a plus
