Job summary

Hillcrest HealthCare System is seeking a Clinical Navigator RN to join our Tulsa team on the day shift. The role coordinates patient care across the continuum and serves as a clinical resource for patients, families, and interdisciplinary teams, promoting high-quality outcomes.

Responsibilities include care coordination, patient education, and implementing clinical program initiatives while upholding evidence-based practices and collaborative care.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license to practice in Oklahoma.
• BLS certification obtained within 14 days of hire.

• Hillcrest Medical Center is a 656-bed hospital that includes The Alexander Burn Center, The Peggy V. Helmerich Women’s Health Center, Kaiser Rehabilitation Center, the latest technology for the treatment of cancer, and the Oklahoma Heart Institute.


• The Clinical Navigator RN coordinates and facilitates patient care across the continuum of care for designated patient populations.


• The position serves as a clinical resource, patient advocate, and liaison among patients, families, providers, and interdisciplinary teams to promote high-quality outcomes and a positive patient experience.


• This role supports care coordination, patient education, clinical program initiatives, and evidence-based practice standards.


• The Clinical Navigator RN contributes to improved clinical outcomes through effective communication, collaboration, and patient-centered care management.

Job Requirements:

• Graduate of an accredited school of nursing


• Current Registered Nurse (RN) license


• BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care

Preferred Job Requirements:

• Bachelor of Science in Nursing (BSN)


• Experience in cardiology, vascular services, critical care, cardiac surgery, or emergency services
