Job summary

Hillcrest HealthCare System in Tulsa, OK invites a Registered Nurse to join as a Population Health Management Clinical Coordinator on day shifts, PRN. You will coordinate clinical, operational, and quality activities to support patient care and organizational goals.

This role acts as a liaison among patients, providers, leadership, and interdisciplinary teams to improve care coordination, regulatory compliance, and performance improvement.

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license.
• 2+ years of clinical nursing experience.

Responsibilities

• The Clinical Coordinator RN coordinates clinical, operational, and quality activities that support patient care services and organizational objectives.
• The position serves as a liaison among patients, providers, leadership, and interdisciplinary teams to facilitate effective care coordination, regulatory complianc e, and performance improvement initiatives.

Job Requirements:

• G raduate of an accredited school of nursing.
• Current Registered Nurse (RN) license.
• 2 + years of clinical nursing experience.

Preferred Job Requirements:

• Bachelor of Science in Nursing (BSN).
• Experience in program coordination, case management, quality improvement, or clinical operations.
• BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.
