Job summary

Hillcrest HealthCare System in Tulsa, OK is seeking a day shift, full-time Lung Center Clinical Navigator RN to coordinate and facilitate patient care across the continuum for designated populations.

The Clinical Navigator RN acts as a clinical resource, patient advocate, and liaison among patients, families, providers, and interdisciplinary teams to promote high-quality outcomes and a positive patient experience. Requirements include RN license and BSN preferred; BLS within 14 days.

Qualifications

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license.
• BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.

Responsibilities

• Coordinates and facilitates patient care across the continuum of care for designated patient populations.
• Serves as a clinical resource, patient advocate, and liaison among patients, families, providers, and interdisciplinary teams.

Job Requirements

• Graduate of an accredited school of nursing.
• Current Registered Nurse (RN) license.

Preferred Qualifications

• Bachelor of Science in Nursing (BSN).
• Experience in cardiology, vascular services, critical care, cardiac surgery, or emergency services.
• Public speaking or community education experience.
• Knowledge of quality improvement me thodologies.
• Skill in program coordination and patient navigation activities.
