Job Summary

Fairport Rehab & Nursing Center is seeking a compassionate and dedicated Registered Nurse (RN) to join our long-term care team. In this role, you will provide high-quality, resident-centered nursing care while promoting the comfort, dignity, safety, and overall well-being of our residents.

You will work collaboratively with nursing staff, providers, residents, families, and other members of the interdisciplinary care team to develop and implement individualized care plans and achieve positive resident outcomes.

Responsibilities
  • Provide direct nursing care to residents in accordance with established care plans, physician orders, and facility policies.

  • Administer medications and treatments safely and accurately.

  • Monitor and document residents' vital signs, health status, and changes in condition.

  • Develop, implement, and evaluate individualized care plans in collaboration with the interdisciplinary team.

  • Participate in rounds, medication passes, assessments, and other routine nursing responsibilities.

  • Communicate changes in residents' conditions to appropriate members of the healthcare team.

  • Collaborate with residents, families, physicians, and other healthcare professionals to address care needs and concerns.

  • Provide education and support to residents and their families regarding health conditions, treatments, and care needs.

  • Maintain accurate, timely, and complete clinical documentation and medical records.

  • Follow infection prevention, safety, and quality assurance standards.

  • Participate in quality improvement initiatives designed to enhance resident care and outcomes.

  • Promote a respectful, supportive, and resident-centered environment.

Qualifications
  • Current and unrestricted Registered Nurse (RN) license in the State of New York.

  • Previous long-term care or skilled nursing experience preferred.

  • Strong clinical assessment, communication, and organizational skills.

  • Ability to work effectively as part of an interdisciplinary team.

  • Compassionate and professional approach to resident care.

  • Ability to prioritize responsibilities and respond appropriately to changes in resident condition.

    Compensation and Schedule:

    • Pay Range: $40.00 – $42.00 per hour with pay commensurate with certification and years of post-License experience

    • Shift Differentials: +$1.50/hr for weekday B & C shifts and +10% more/hour for all weekend shifts

    • Available Shifts: Day (7:00am – 3:00pm), Evening (3:00pm – 11:00pm) Nights (11:00pm-7:00AM)

    • Schedule Options: Full-Time (37.5 hours per week), Part-Time (22.5 hours per week)

    • Weekend Availability: Every other weekend Requirment

    • Sign-On Bonus: $4,000 Sign-On Bonus for Full-Time RN's
