Job summary

Optum Home & Community Care in Warrenton, VA area is seeking a Registered Nurse to deliver skilled home health nursing, build lasting relationships with patients, and help manage chronic conditions in the comfort of their homes. You’ll work with a supportive care team and gain meaningful, hands-on patient impact.

You will assess needs, develop care plans, educate patients and families, collaborate with a multidisciplinary team, and document care accurately while enjoying autonomy and flexible

Required Qualifications

The salary range for this role is $82,995 - $124,492 annual total cash target pay based on full-time employment and may vary based on factors including, but not limited to, local labor markets, education, work experience, and certifications. Compensation and benefits information will be discussed during the hiring process.
