Job summary

UnitedHealth Group is seeking a Registered Nurse for home health in Bradenton, Florida. You will coordinate with physicians and healthcare professionals to develop and revise plans of care and ensure seamless patient transitions.

In this role you will supervise home health aides and LPNs, perform supervisory visits, complete OASIS assessments, and ensure accurate documentation for payer requirements while maintaining infection control standards.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience preferred.
• Current Driver's License and vehicle insurance required.
• Current CPR Certification or ability to complete within 90 days of hire preferred.

Responsibilities

• Initiate, develop, implement, and revise the plan of care in collaboration with the physician and other healthcare professionals.
• Supervise care provided by home health aides and LPN/LVN, providing instruction and assigning tasks.
• Provide required supervisory visits.
• Provide high-quality clinical services within scope of practice and infection control standards.
• Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Document patient visits per policy and payer requirements.
• Make initial and/or comprehensive nursing evaluation visits to ensure home health eligibility.
• Communicate relevant information timely and effectively with appropriate agency staff.
• Assist in the orientation of new agency personnel and serve as a preceptor to other staff and students.
• Participate in on-call and weekend rotation as needed to meet patient needs.
