Brighton/Suncrest Hospice is seeking an RN to provide compassionate hospice care across patient home settings, including private residences, SNFs, and assisted living facilities. You will work with an interdisciplinary team to tailor care plans that address physical, emotional and spiritual needs.

The role emphasizes lower clinician caseloads to spend meaningful time with patients and families, with opportunities for growth within the organization.

Qualifikationen

• Current Registered Nurse (RN) license in good standing in the state of practice.
• Passion for providing exceptional, patient-centered hospice care.
• Strong collaboration and communication with interdisciplinary teams.
• Ability to work independently while managing daily schedule.
• Excellent time management and organizational skills for patient visits, documentation, meetings, and urgent needs.
• Familiarity with electronic medical record (EMR) systems.

You will collaborate with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans that support the physical, emotional, and spiritual needs of patients and their families. This allows our nurses to spend more meaningful time with patients and families, build lasting relationships, and provide the compassionate, personalized care every patient deserves. • Reliable transportation, a valid driver's license, and current auto insurance to travel throughout the service area
