Job summary

Suncrest Hospice is expanding its compassionate care team to deliver high-quality hospice services across homes and facilities in Texas. As an RN, you will provide individualized, patient- and family-centered hospice care in patients' homes, skilled nursing facilities, and assisted living communities.

You will collaborate with an interdisciplinary team of nurses, social workers, chaplains, CNAs, and volunteers to build and adjust care plans, prioritizing quality care, communication, and patient

Qualifications

• Current RN license in good standing in the state of practice.
• Strong collaboration and communication with interdisciplinary teams.
• Ability to work independently while managing daily schedule.
• Excellent time management and organizational skills.
• Comfort learning and using EMR systems.
• Reliable transportation, a valid driver's license, and current auto insurance to travel throughout the service area.

Responsibilities

• Provide hospice care at patients' homes, including private residences and facilities.

• Maintain patient visits, documentation, and urgent needs while balancing caseloads.

As a Registered Nurse (RN), you'll provide compassionate, individualized hospice care wherever patients call home-including private residences, skilled nursing facilities, and assisted living communities.

You will collaborate with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans that support the physical, emotional, and spiritual needs of patients and their families. We prioritize quality over quantity by maintaining lower clinician caseloads and higher staff-to-patient ratios. This allows our nurses to spend more meaningful time with patients and families, build lasting relationships, and provide the compassionate, personalized care every patient deserves.
