Job summary

Brighton/Suncrest Hospice is seeking an experienced Registered Nurse (RN) to provide compassionate hospice care in patients' homes, skilled nursing facilities, and assisted living communities across the service area.

You will collaborate with an interdisciplinary team to develop and implement individualized care plans, ensuring high-quality, patient-centered care while maintaining manageable caseloads and strong caregiver support.

Qualifications

• Current Registered Nurse license in good standing in the state of practice.
• Commitment to patient-centered hospice care.
• Strong collaboration and communication with interdisciplinary teams.
• Ability to work independently while managing daily schedule.
• Excellent time management and organization to balance visits and documentation.
• Familiarity with EMR systems.
• Reliable transportation and valid driver's license to travel the service area.

Responsibilities

• Provide compassionate hospice care in home settings and facilities.
• Collaborate with nurses, social workers, chaplains, CNAs and volunteers to develop care plans.
• Maintain lower clinician caseloads and higher staff-to-patient ratios to ensure time with patients.
• Manage visits, documentation, and urgent needs with strong organizational skills.

As a Registered Nurse (RN), you'll provide compassionate, individualized hospice care wherever patients call home—including private residences, skilled nursing facilities, and assisted living communities.

You will collaborate with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans that support the physical, emotional, and spiritual needs of patients and their families. We prioritize quality over quantity by maintaining lower clinician caseloads and higher staff-to-patient ratios. This allows our nurses to spend more meaningful time with patients and families, build lasting relationships, and provide the compassionate, personalized care every patient deserves.
