Job summary

Brighton/Suncrest Hospice is seeking an experienced Registered Nurse (RN) to deliver compassionate hospice care in patients' homes and settings across the service area. You’ll collaborate with nurses, social workers, chaplains, CNAs, and volunteers to plan and execute individualized care plans.

You will spend meaningful time with patients and families, balancing visits, documentation, and urgent needs while maintaining lower clinician caseloads.

Qualifications

• Current Registered Nurse (RN) license in good standing in the state of practice.
• A passion for providing exceptional, patient-centered hospice care.
• Excellent collaboration and communication with interdisciplinary teams.
• Ability to work independently while effectively managing your daily schedule.
• Excellent time management and organizational skills for visits, documentation, meetings, and urgent needs.
• Reliable transportation, valid driver's license, and auto insurance to travel throughout the service area.

Responsibilities

• Provide compassionate, individualized hospice care wherever patients call home.
• Collaborate with an interdisciplinary team to develop and adjust care plans.
• Manage daily schedule, patient visits, and timely documentation in EMR.
