Job summary

Brighton/Suncrest Hospice is seeking an experienced Registered Nurse to provide compassionate hospice care in patients' homes and care facilities. You will work with an interdisciplinary team to develop and carry out individualized care plans, ensuring quality and patient-centered service.

Ideal candidates display strong communication, time management, and organizational skills, with the ability to travel within the service area and manage a balanced schedule with documentation and visits.

Qualifications

• Current RN license in good standing in the state of practice.
• A passion for providing exceptional, patient-centered hospice care.
• Strong collaboration and communication skills with interdisciplinary care teams.
• Ability to work independently while effectively managing your daily schedule.
• Excellent time management and organizational skills to balance patient visits, documentation, meetings, and urgent needs.
• Comfort learning and utilizing electronic medical record (EMR) systems.
• Reliable transportation, a valid driver's license, and current auto insurance to travel throughout the service area.

Responsibilities

• Provide compassionate, individualized hospice care wherever patients call home—homes, SNFs, or assisted living communities.
• Collaborate with an interdisciplinary team of nurses, social workers, chaplains, CNAs, and volunteers to develop and implement care plans.
• Maintain lower clinician caseloads and higher staff-to-patient ratios to ensure time with patients and families.
• Support quality, patient-centered care with strong communication and documentation practices.
• Travel within the service area to deliver timely hospice services as needed.

You will collaborate with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans that support the physical, emotional, and spiritual needs of patients and their families. We prioritize quality over quantity by maintaining lower clinician caseloads and higher staff-to-patient ratios. This allows our nurses to spend more meaningful time with patients and families, build lasting relationships, and provide the compassionate, personalized care every patient deserves.
