Qualifications

• Current Registered Nurse (RN) license in good standing in the state of practice.
• Commitment to providing exceptional, patient-centered hospice care.
• Strong collaboration and communication with interdisciplinary care teams.
• Ability to work independently while effectively managing daily schedule.
• Excellent time management and organizational skills.
• Comfort learning and utilizing electronic medical record (EMR) systems.
• Reliable transportation with valid driver's license and insurance for travel within the service area.

Responsibilities

• Provide compassionate, individualized hospice care wherever patients call home—private residences, SNFs, and ALFs.
• Collaborate with an interdisciplinary team of RNs, social workers, chaplains, CNAs, and volunteers to develop care plans.
• Balance patient visits, documentation, meetings, and urgent needs while maintaining high-quality care.

As a Registered Nurse (RN), you'll provide compassionate, individualized hospice care wherever patients call home-including private residences, skilled nursing facilities, and assisted living communities.

You will collaborate with an interdisciplinary team of nurses, Social Workers, Chaplains, CNAs, and volunteers to develop care plans that support the physical, emotional, and spiritual needs of patients and their families. We prioritize quality over quantity by maintaining lower clinician caseloads and higher staff-to-patient ratios. This allows our nurses to spend more meaningful time with patients and families, build lasting relationships, and provide the compassionate, personalized care every patient deserves.
