Job summary

Select Home Health & Hospice (A Fortis Health Company) in Indianapolis seeks a skilled Registered Nurse – Admissions to complete patient admission visits across the area. This floating role supports both home health and hospice services with initial assessments and care planning.

Responsibilities include ongoing patient assessments, developing plans of care with physicians, and completing clinical documentation.

Qualifications

• Graduate of an accredited school of professional nursing.
• Current Indiana RN license.
• Minimum of 1 year nursing experience in a hospital, facility, or home health setting; home health experience preferred.

Responsibilities

• Complete patient admission visits and initial assessments for home health and hospice patients.
• Perform patient assessments and ongoing evaluations to monitor patient needs and changes in condition.
• Develop and implement individualized plans of care in collaboration with physicians and the clinical team.
• Complete required clinical documentation, including OASIS assessments for eligible home health patients.
• Provide in-home nursing care and additional patient visits as needed based on agency needs.
• Educate patients and caregivers on medications, diagnoses, treatment plans and in-home care.
• Coordinate patient services and communicate changes in patient condition to physicians and the appropriate care team.
• Float throughout the Indianapolis area to provide admission coverage and additional nursing support as needed.
