Job summary

Human Touch Home Health is seeking an experienced and compassionate Registered Nurse (RN) to join our growing team serving patients throughout Richmond, Henrico, Chesterfield, Mechanicsville, Glen Allen, Midlothian, Hanover, and surrounding communities.

As a PRN Home Health RN, you'll provide individualized in-home care, educate patients and families, manage medications and wounds, and coordinate with therapists and the care team to prevent hospitalizations and support independence.

Qualifications

• Active Virginia RN license required.
• Valid driver's license and reliable transportation.
• Strong clinical assessment and documentation skills.
• Excellent communication and organizational skills.
• Home Health experience preferred but not required.
• Experience with OASIS, wound care, case management or chronic disease management is a plus.

Responsibilities

• Perform comprehensive patient assessments.
• Develop and implement individualized plans of care.
• Provide skilled nursing services, medication management, wound care, and patient education.
• Monitor patient progress and communicate changes to physicians and the interdisciplinary team.
• Coordinate care with therapists, social workers, and home health aides.
• Complete timely and accurate clinical documentation.
• Support patients and families throughout their healthcare journey.
