Human Touch Home Health is seeking Registered Nurses (RNs) for full-time and PRN home health opportunities throughout Washington, DC. Join a team that values one-on-one patient care, independence, and clinical skill application outside of traditional facilities.

Qualifications

• Active Washington, DC Registered Nurse License Required.
• Graduate of an accredited nursing program.
• Current CPR/BLS Certification.

Responsibilities

• Complete skilled nursing assessments and home visits.
• Perform Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned.
• Develop and update individualized plans of care.
• Provide wound care, medication management, and disease education.
• Monitor patient conditions and report significant changes.
• Educate patients and caregivers.
• Coordinate care with physicians, therapists, and other team members.
• Complete accurate and timely documentation using WellSky.
• Help prevent unnecessary hospitalizations and support patients safely at home.

What You'll Do
