Job summary

Human Touch Home Health seeks an experienced Home Health RN to serve patients in Richmond and surrounding communities. This PRN role offers flexible scheduling and the opportunity to build meaningful relationships with patients in the comfort of their homes.

As a Home Health RN, you will deliver individualized care, educate families, and coordinate with therapists, physicians, and aides to help patients recover, manage chronic conditions, and maintain independence.

Qualifications

• Active Virginia Registered Nurse (RN) License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment and Documentation Skills
• Excellent Communication and Organizational Skills
• Home Health Experience Preferred but Not Required
• Experience with OASIS, wound care, case management, or chronic disease management is a plus

Responsibilities

• Perform comprehensive patient assessments
• Develop and implement individualized plans of care
• Provide skilled nursing services, medication management, wound care, and patient education
• Monitor patient progress and communicate changes to physicians and the interdisciplinary team
• Coordinate care with therapists, social workers, and home health aides
• Complete timely and accurate clinical documentation
• Support patients and families throughout their healthcare journey
