Job summary

Human Touch Home Health is seeking registered nurses (RNs) to join our Blacksburg team in full-time and PRN roles, delivering skilled home health care across Blacksburg and nearby communities.

You will perform initial and follow-up visits, develop care plans, administer wound care and medications, educate patients and families, and coordinate with physicians and therapists to keep patients safely at home. Competitive benefits and supportive leadership available.

Qualifications

• Active Virginia RN license required.
• Graduate of an accredited nursing program.
• Current CPR/BLS certification.
• Valid Driver's License and reliable transportation.
• WellSky experience is a plus.

Responsibilities

• Complete skilled nursing assessments and home visits.
• Perform Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned.
• Develop and update individualized plans of care.
• Provide wound care, medication management, and disease education.
• Monitor patient conditions and communicate significant changes.
• Educate patients and caregivers.
• Coordinate care with physicians, therapists, and other team members.
• Complete accurate and timely documentation using WellSky.
• Help patients safely manage their health at home.

Human Touch Home Health is seeking compassionate and dependable Registered Nurses (RNs) for Full-Time and PRN opportunities with our Blacksburg team.

You'll provide skilled nursing care to patients throughout Blacksburg, Christiansburg, Radford, Pulaski, Floyd, and surrounding New River Valley communities.
