Job summary

Human Touch Home Health in Blacksburg, VA is seeking compassionate and dependable Registered Nurses (RNs) for Full-Time and PRN opportunities. You will provide skilled nursing care to patients across Blacksburg, Christiansburg, Radford, Pulaski, Floyd, and surrounding New River Valley communities.

Qualifications

• Active Virginia Registered Nurse (RN) License is required.
• Graduate of an accredited nursing program.
• Current CPR/BLS Certification.
• Valid Driver's License and reliable transportation.
• Strong clinical assessment and communication skills.
• Ability to work independently in patients' homes.
• Home Health and OASIS experience preferred.
• WellSky experience is a plus.

Responsibilities

• Complete skilled nursing assessments and home visits.
• Perform Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned.
• Develop and update individualized plans of care.
• Provide wound care, medication management, and disease education.
• Monitor patient conditions and communicate significant changes.
• Educate patients and caregivers.
• Coordinate care with physicians, therapists, and other team members.
• Complete accurate and timely documentation using WellSky.
• Help patients safely manage their health at home.
