Job summary

Rehabilitation Hospital of Oak Creek, part of Lifepoint Health, seeks a registered nurse to deliver high-quality inpatient rehabilitation care in a brand-new 40-bed facility. The role emphasizes patient assessment, family teaching, and coordination with an interdisciplinary team to maximize recovery and independence.

This PRN RN position requires BLS within 30 days, Wisconsin RN license, an associate degree, and at least one year of experience.

Qualifications

• Certification: AHA Basic Life Support (BLS) within 30 days of hire.
• Requires critical thinking, decisive judgment and ability to work with minimal supervision.
• One year of previous experience preferred.
• Active Wisconsin or compact state RN license required (PSOR updates if relocating).
• Associate’s Degree required.

Responsibilities

• Perform accurate patient assessments and identify needs.
• Identify and initiate appropriate nursing interventions.
• Provide patient and family teaching during rehabilitation and prior to discharge.
• Deliver care appropriate to patient condition and age.
• Collaborate with interdisciplinary team.
• Participate in CQPI activities.
• Complete and revise Interdisciplinary Care Plans for each patient.
• Perform timely QI assessments.

PRN role – Shifts are 12 hours and will need to commit to 2 shifts every 6 weeks as a minimum

Rehabilitation Hospital of Oak Creek is a brand-new 40 bed inpatient rehabilitation facility. We offer high‑quality, compassionate care for a wide variety of patients in need. We strive to maximize the health, function, and quality of life of those we serve through comprehensive physical medicine and rehabilitation programs.

How You’ll Contribute

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
• Provides care appropriate to condition and age of the patient.
• Works cooperatively and actively participates with other members of the interdisciplinary team.
• Participates in Continuous Quality and Performance Improvement (CQPI).
• Performs timely and appropriate documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.

What We Are Looking For

• Certifications: AHA Basic Life Support (BLS) within 30 days of hire
• Required Skills: Requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
• Experience: One year previous experience preferred
• Licenses: Active Wisconsin or compact state RN license required. If relocating, licensure must be updated to new Primary State of Residence (PSOR) before employment begins.
• Education: Associate’s Degree required.
