Job summary

Rehabilitation Hospital of Oak Creek, part of Lifepoint Health, seeks a Licensed Case Manager to coordinate interdisciplinary care plans and discharge processes in a brand-new inpatient rehab setting opening in 2026.

Role involves collaboration with patients, families, and the healthcare team to ensure timely transitions. Requires licensure in a related field and 2+ years in inpatient case management or social work.

Qualifications

• Licensure required as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
• Certifications in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
• Minimum of 2 years of experience in social work or case management in an inpatient setting; acute rehabilitation hospital experience strongly preferred.

Responsibilities

• Coordinate interdisciplinary care plan, discharge planning and patient progress communication within inpatient rehabilitation.
• Identify barriers to care and discharge with the team and develop solutions.
• Document patient care activities per CMS guidelines and IPoC.
• Schedule family conferences and communicate progress and discharge planning with caregivers.
• Coordinate weekly interdisciplinary team conferences to monitor goals and outcomes.
• Review CMG and communicate findings to HIM team.

How you'll contribute

The Licensed Case Manager is responsible for coordinating the interdisciplinary care plan, discharge planning, and patient progress communication within the inpatient rehabilitation setting. This role serves as a liaison among patients, caregivers, and the healthcare team to ensure timely and effective care transitions.

• Collaborates with interdisciplinary team members to identify barriers to care or discharge and develops appropriate solutions.
• Documents patient care activities in alignment with workflow timelines, including completion of the Individual Plan of Care (IPoC) per CMS guidelines.
• Schedules family conferences and communicates progress and discharge planning with caregivers after team conferences and as needed.
• Coordinates weekly interdisciplinary team conferences to monitor treatment goals and outcomes.
• Reviews assigned Case Mix Group (CMG) and assists the team in identifying actively treated comorbid conditions; communicates findings to the Health Information Management (HIM) team.

What we're looking for

• Education & License: Licensure required as a Registered Nurse, Social Worker, Respiratory Therapist, Physical Therapist, Occupational Therapist, or Speech-Language Pathologist.
• Certifications: Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ACM, RN-BC, ARN).
• Experience: Minimum of 2 years of experience in social work or case management in an inpatient setting preferred; acute rehabilitation hospital experience strongly preferred.
• Skills: Effective oral and written communication in English; additional languages preferred. Proficiency in Microsoft Office applications required.
