Job summary

Rehabilitation Hospital of Oak Creek, part of Lifepoint Health, is seeking a Registered Nurse to join our brand-new 40-bed inpatient rehabilitation facility in Oak Creek, Wisconsin. This full-time night shift role offers a sign-on bonus up to $15,000 and opportunities to deliver compassionate, interdisciplinary care.

As a team member, you will assess patients, plan care with the interdisciplinary team, provide patient and family education, and document care accurately to support excellent

Qualifications

• BLS certification within 30 days of hire.
• Minimum of one year of previous nursing experience preferred.
• Active Wisconsin RN license or compact state license.
• Associate's degree in nursing required.

Responsibilities

• Accurately performs patient assessments and identifies patient needs.
• Identifies and initiates appropriate nursing interventions.
• Provides patient and family teaching throughout the rehabilitation stay and prior to discharge.
• Provides care appropriate to condition and age of the patient.
• Works cooperatively with other members of the interdisciplinary team.
• Participates in Continuous Quality and Performance Improvement (CQPI).
• Performs timely documentation relating to medical necessity in the medical record.
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient.
• Performs timely and accurate QI assessments.

How You'll Contribute

A Registered Nurse who excels in this role:

What We Are Looking For

• Certifications: AHA Basic Life Support (BLS) within 30 days of hire
• Required Skills: Requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
• Experience: One year previous experience preferred
• Licenses: Active Wisconsin or compact state RN license required. If relocating, licensure must be updated to new Primary State of Residence (PSOR) before employment begins.
• Education: Associate's Degree required.
