Job summary

Rehabilitation Hospital of Oak Creek is looking for a dedicated Case Manager to support inpatient rehabilitation teams and discharge planning. You will coordinate care, document care plans, and communicate progress with families and caregivers.

Minimum 2 years in social work or case management in an inpatient setting is preferred; relevant certifications are a plus. Per diem coverage during days ensures flexible scheduling in a new 40-bed facility opening in 2026.

Qualifications

• Education: Current RN or Social Work licensure or Healthcare professional licensure as Rehabilitation Therapist, PT, OT or SLP.
• Experience: Minimum of 2 years' social work or case management in inpatient settings preferred.
• Licensure: CCM/ARN/ACM or RN-BC certifications preferred.

• Completes departmental orientation and annual competencies.
• Collects and reports data for performance improvement initiatives.
• Collaborates with care team to address barriers to care and discharge planning.
• Completes IPoC per CMS guidelines and documents timely.
• Schedules family conferences and maintains progress updates with caregivers.
• Coordinates weekly care team conferences to refine treatment plans.
• Reviews CMG and communicates findings to HIM and team.
• Communicates effectively with nursing, therapy and ancillary departments.

How you'll contribute

A Case Manager who excels in this role:

• Completes departmental orientation, initial and annual competencies.
• Assists with departmental specific performance improvement initiatives collecting and reporting data as requested by supervisor.
• As appropriate, consults other departmental staff to collaborate in patient care delivery, identify barriers to care and or discharge and develop solutions/resolution.
• Completes documentation per workflow timeline and content requirements including completion of the Individual Plan of Care (IPoC) per CMS guidelines.
• Schedules family conferences and/or communicates with caregiver following each team conference and more often as needed to keep patient and designated caregiver informed of progress and provides appropriate information related to goal achievement, course of rehabilitation stay, and plans for discharge.
• Coordinates weekly patient care team conferences to facilitate development, monitoring and refinement of treatment plans to achieve identified patient goals and outcomes.
• Reviews the patient's assigned CMG and helps the team identify any potential missed comorbid conditions that are actively being treated during the patient's stay. Communicate any findings to the HIM team.
• Communicates effectively with nursing, therapy and other ancillary departments to ensure proper utilization.

What we're looking for

Applicants should possess the following qualifications and skills:

• Education: Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist or Occupational Therapist.
• Experience: Minimum of 2 years' social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
• License: Certification in Case Management or Rehabilitation Nursing preferred; for example, Commission for Case Manager Certification (CCM); Association of Rehabilitation Nurses (ARN) certification, American Case Management Association (ACM) or Board Certification in CM by the ANCC e.g.: RN-BC.
