Job summary

Homewatch CareGivers of Woodbridge is seeking an RN Care Manager to design person-centered in-home care plans, mentor caregivers, and ensure safe, quality home care for seniors.

You will lead assessments, craft plans with families, and coordinate with physicians to keep clients healthy at home while enjoying autonomy and support in a flexible schedule.

Qualifications

• Active RN license in NJ.
• Bachelor’s in Nursing or related field, or 2+ years in geriatric/home-care.
• Strong clinical assessment skills and problem-solving.
• CPR/First Aid certification and valid driver’s license.
• Warm, professional communication with families and field staff.

Responsibilities

• Perform in-home RN assessments and craft individualized care plans with client/family approval.
• Coach and supervise caregivers, conduct field competency checks, ensure compliance.
• Monitor vitals and health changes; document in HIPAA-secure EMR and escalate concerns.
• Coordinate with physicians, therapists, and schedulers to ensure seamless, client-centered care.
• Maintain audit-ready files and ensure compliance with state home-care regulations.

What You’ll Bring
