Job summary

Attend Home Care is seeking an experienced RN Case Manager to deliver high-quality home health care on a part-time/per-diem basis in Dayton, Ohio. You will conduct SOC visits, provide skilled nursing services, and educate patients and caregivers while coordinating with physicians and the care team.

Qualifications

• Active RN license in the state of practice.
• Experience in home health, skilled nursing, or related setting preferred.
• Strong clinical assessment and critical thinking skills.
• Knowledge of home health documentation standards.

Responsibilities

• Conduct SOC visits to complete comprehensive assessments and develop physician-directed care plans.
• Perform skilled nursing visits and ongoing reassessments to monitor and adjust care.
• Provide hands-on skilled nursing care including wound care, medications, injections, and chronic disease management.
• Educate patients and caregivers on diagnoses, medications, and preventive care to reduce hospitalizations.
• Collaborate with physicians and interdisciplinary team to ensure coordinated, high-quality care.
• Maintain accurate and timely clinical documentation in compliance with regulations and standards.
• Supervise and delegate care to LPNs and Home Health Aides as per the care plan.

Employment Type: Part-Time/ Per-Diem (Flexible Scheduling Available)

Compensation: Competitive Pay, PRN: $125 SOC Visit / $75 Reassessment Visit + Milegae

What You Will Do:

• Conduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop individualized, physician-directed care plans
• Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed
• Provide hands-on skilled nursing care , including wound care, medication management, injections, and chronic disease management
• Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations
• Collaborate with physicians and interdisciplinary team members to ensure coordinated, high-quality care
• Maintain accurate and timely clinical documentation in compliance with state regulations and agency standards
• Supervise and delegate care to LPNs and Home Health Aides to ensure quality and adherence to the plan of care

What You Will Have:

• Active Registered Nurse (RN) license in the state of practice
• Experience in home health, skilled nursing, or a related healthcare setting preferred
