Homewatch CareGivers in New Jersey is seeking an experienced Registered Nurse to join as RN Care Manager for Home Care. You’ll design person-centered plans of care, mentor caregivers, and champion quality that keeps seniors safe and comfortable at home.

This role offers a flexible schedule, autonomy with support, and opportunities for CEU reimbursement and growth toward Nurse Supervisor or Clinical Director within a collaborative, patient-focused culture.

Qualifications

• Active RN license required.
• Bachelor’s in Nursing or related field, or 2+ years’ geriatric/home-care experience.
• Solid clinical assessment skills and a passion for problem-solving.
• CPR/First Aid certification and a valid driver’s license.
• Warm, professional communication that builds trust with families and field staff.

Responsibilities

• Complete in-home RN assessments; craft individualized Care Plans with client/family approval.
• Orient caregivers to each Care Plan, perform field competency checks, and ensure ongoing compliance.
• Track vitals, condition changes, and interventions in HIPAA-secure EMR; elevate concerns promptly.
• Coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
• Maintain audit-ready client files, collect initial service payments, and uphold state home-care regulations.
