Job summary

Akin Care Senior Services seeks an RN Care Manager to design person-centered plans of care and mentor caregivers, ensuring seniors stay safely at home.

You’ll assess clients in their homes, coordinate with physicians, therapists, and family members, and supervise field staff to uphold quality and compliance. A valid NJ RN license and geriatric/home-care experience are required for this role.

Qualifications

• Minimum in-home or geriatric care experience.
• Ability to design and approve personalized care plans.
• Strong coaching and supervision of caregivers.
• Maintain HIPAA-compliant documentation.

Responsibilities

• Assess and plan—develop individualized care plans with client/family approvals.
• Coach and supervise caregivers; ensure field competency.
• Monitor vitals, changes in condition, and interventions.
• Collaborate with physicians, therapists, and schedulers for seamless care.
• Maintain audit-ready files and ensure regulatory compliance.

How You’ll Make an Impact

• Assess & Plan
  • Complete in-home RN assessments; craft individualized Care Plans with client/family approval.
• Coach & Supervise
  • Orient caregivers to each Care Plan, perform field competency checks, and ensure ongoing compliance.
• Monitor & Document
  • Track vitals, condition changes, and interventions in our HIPAA-secure EMR; elevate concerns promptly.
• Collaborate & Advocate
  • Coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
• Quality & Compliance
  • Maintain audit-ready client files, collect initial service payments, and uphold state home-care regulations.

What You’ll Bring

• Active Registered Nurse (RN) license in New Jersey
• Bachelor’s in Nursing or related human-services field – OR – 2+ years’ geriatric/home-care experience
• Solid clinical assessment skills and a passion for problem-solving
• CPR/First Aid certification and a valid driver’s license
• Warm, professional communication that builds trust with families and field staff
