Job summary

Homewatch CareGivers is seeking an experienced Registered Nurse (RN) Care Manager to design person-centered home care plans, mentor caregivers, and advocate for seniors’ comfort and safety at home.

You will perform in-home assessments, coordinate with families and clinicians, and lead with clinical expertise and compassion to ensure high-quality, client-centered care.

Qualifications

• Active Registered Nurse (RN) license required.
• Bachelor’s in Nursing or related human-services field – OR – 2+ years’ geriatric/home-care experience.
• CPR/First Aid certification and a valid driver’s license.
• Warm, professional communication that builds trust with families and field staff.

Responsibilities

• Assess and plan: complete in-home RN assessments; craft individualized care plans with client/family approval.
• Coach and supervise: orient caregivers to each Care Plan, perform field competency checks, ensure ongoing compliance.
• Monitor and document: track vitals, condition changes, and interventions in HIPAA-secure EMR; elevate concerns promptly.
• Collaborate and advocate: coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
• Quality and compliance: maintain audit-ready client files, uphold state home-care regulations.
