Responsibilities

The Registered Nurse in Home and Community Based Services provides and directs nursing and personal care to patients in their homes, following physician prescriptions, state/program authorizations, laws, regulations, and agency policies. The RN coordinates care plans with other healthcare professionals and ensures continuity of care by communicating with physicians, agency staff, patients, families, case managers, and community resources.

Education and Experience

Deliver high-quality clinical services within your scope of practice and infection control standards, in accordance with the care plan and in coordination with the care team from admission to discharge. Complete clinical nursing assessments as required by federal/state programs and payers. Ensure patient eligibility and medical necessity for services. Develop and revise individualized care plans with community providers. Ensure care plan frequency and duration meet patient needs and revise plans with physician approval.

Experience and License Requirements

• Minimum of one year RN experience; PEDS experience preferred
• Current RN licensure in the practicing state
• Valid driver’s license, vehicle insurance, access to a dependable vehicle or public transportation
• Current CPR certification
