Job summary

Ascension Health is seeking a dedicated Registered Nurse for the Seton Lockhart Health Center in Lockhart, TX. You will lead individualized care plans, coordinate multidisciplinary teams, and ensure safe, evidence-based patient care throughout hospitalization and discharge planning.

The role emphasizes ensuring patient safety, communicating clinical changes in real time, and delivering high-quality nursing care per the organization’s standards.

Qualifications

  • RN licensure in Texas.
  • BLS certification within 1 month of hire.
  • Ambulatory or long-term care clinical experience preferred.

Responsibilities

  • Lead implementation and monitoring of individualized care plans for safe, evidence-based treatment.
  • Coordinate multidisciplinary teams as primary contact for holistic patient recovery.
  • Interpret physician orders accurately and communicate changes to the care team.
  • Navigate discharge planning to ensure a clear pathway home.
  • Provide direct nursing care while adhering to safety and organizational standards.

  • Schedule: Full-time | Monday-Friday 8am-5pm

How you'll make an impact in this role
  • Lead the implementation and continuous monitoring of individualized care plans to ensure every patient receives safe, evidence-based treatment.

  • Coordinate multidisciplinary teams as the primary point of contact, ensuring seamless communication across all specialties for holistic patient recovery.

  • Advocate for patient safety by accurately interpreting physician orders and communicating real-time clinical changes to the care team.

  • Navigate the discharge planning process in collaboration with the healthcare team to ensure patients have a clear, supported path for their transition home.

  • Champion clinical excellence by delivering direct nursing care that adheres to the highest organizational standards and safety protocols.

What minimum requirements you'll need
Licensure / Certification / Registration:
  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

  • Licensed Registered Nurse credentialed from the Texas Board of Nursing required.

Education:
  • Diploma from an accredited school/college of nursing required OR a bachelors level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed.
Additional Preferences:
  • Clinical Qualifications & Experience: Holds an active Registered Nurse license with background in Internal or Family Medicine, alongside hands-on proficiency using the Athena EMR platform.

  • Core Competencies & Capabilities: Demonstrates strong customer service, problem-solving, and multitasking abilities, with the independence to work autonomously and flexibility to travel as needed.
