Ascension Health is seeking a dedicated Registered Nurse for Saint Thomas Medical Partners Nashville. The role focuses on comprehensive patient care in a fast-paced primary care setting, with full-time daytime hours.

You will manage discharge planning, monitor patient conditions, and accurately carry out physician orders while coordinating with the care team to support safe transitions home. A TN RN license and BLS certification are required.

Qualifications

• Registered Nurse credentialed from the Tennessee Board of Nursing required prior to hire date.
• BLS Provider certification obtained within 1 month of hire date; American Heart Association or American Red Cross accepted.

• Advocate & Strategist: You will bring patient care plans to lifeâimplementing, monitoring, and evolving them to meet the unique needs of those in your care.

• Vigilant Communicator: Beyond just "recording" data, you will be the expert voice for your patients, monitoring conditions and communicating vital updates to the team in real-time.

• Precision Execution: With a keen eye for detail, you'll interpret and carry out physician and nursing orders, ensuring clinical accuracy at every turn.

• Bridge to Home: You'll lead the discharge planning process from day one, collaborating across the care team to ensure your patients have everything they need for a safe and confident transition home.

What minimum requirements you’ll need

Licensure / Certification / Registration

• Required Credential(s):

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.

• Registered Nurse credentialed from the Tennessee Board of Nursing obtained prior to hire date or job transfer date.

Education

• Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
