Ascension Health at Saint Thomas Brentwood Primary Care in Brentwood, TN is seeking a dedicated Licensed Practical Nurse for a full-time day shift. You will perform holistic patient assessments, deliver treatments, and monitor for adverse reactions to ensure safety and comfort during care.

You will also conduct essential lab tests and provide clear discharge education to patients and families, supporting recovery and continuity of care in a dynamic outpatient setting.

Qualifications

• BLS certification within 30 days of hire.
• LPN credentialed from the Tennessee Board of Nursing or a valid multi-state license (Compact State) before hire.

How you’ll make an impact in this role

• Facilitate holistic patient assessments to identify immediate needs and ensure a personalized, effective care plan is established from the start.

• Deliver precise clinical treatments and medications, vigilantly monitoring for adverse reactions to guarantee patient safety and comfort throughout their stay.

• Perform essential diagnostic lab tests and therapeutic services that provide the clinical team with the critical data needed for timely medical decisions.

• Empower patients and families through education, translating complex treatment plans into clear self-care instructions to ensure a successful recovery after discharge.

What minimum requirements you’ll need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.

• Licensed Practical/Vocational Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date required.

Additional Preferences

• Must be willing to work Monday-Thursday 7am-5:30pm or Monday-Friday 8am-5pm
