Job summary

Ascension’s FCA Primary Care Clinic in Kokomo, IN seeks a Registered Nurse for PRN coverage. You’ll prepare patients for exams, support providers during outpatient procedures, and help coordinate care across referrals and follow-ups.

This role emphasizes clear patient education and precise documentation of progress to support long-term wellness. Eligible candidates hold an Indiana RN license, a nursing diploma, and BLS certification within 30 days of hire.

Qualifications

• Must hold a Registered Nurse license issued by the Indiana Board of Nursing.
• Diploma from an accredited school/college of nursing or equivalent licensure.
• BLS Certification obtained within 30 days of hire date.

Responsibilities

• Lead the clinical patient flow by preparing patients for examinations and assisting providers with outpatient procedures.
• Coordinate referrals, follow-ups, and specialty communication to bridge care between clinic and outside providers.
• Translate physician orders into clear patient education plans to empower home health management.
• Accurately execute clinical orders and document patient progress for long-term care records.

How you’ll make an impact in this role

• Lead the clinical patient flow by preparing patients for examinations and assisting providers with outpatient procedures to ensure a seamless, high-quality visit.
• Coordinate the continuum of care by managing referrals, follow-ups, and specialty communication to bridge the gap between the clinic and outside providers.
• Support patient health literacy by translating complex physician orders into clear, actionable education plans that empower patients to manage their health at home.
• Streamline treatment implementation by accurately executing clinical orders and documenting patient progress to maintain an airtight record of their long-term care journey.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• Required Credential(s):
  • Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date. Valid state license required specific to the state in which the associate works.
  • One or more of the following:
    • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
    • Instructor specializing in Basic Life Support obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.

Education

• Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
