Ascension Health in Carmel, IN is seeking a registered nurse for a Primary Care Phone Triage role. You will perform rapid clinical assessments by phone or in person, prioritizing urgency and directing patients to appropriate care to ensure safety and optimize clinic flow.

The position requires triage/clinic experience, strong communication, listening, and critical thinking skills, with a focus on coordinating care and patient transitions within a multidisciplinary team.

Competenze

• Registered Nurse licensed in Indiana prior to hire.
• Diploma from an accredited nursing program.
• BLS within 30 days; instructor certification preferred.

• Schedule: Monday - Friday 8:00 am - 4:30 pm

How you’ll make an impact in this role

• Perform rapid clinical assessments over the phone or in person, using expert critical thinking to accurately prioritize patient needs based on urgency.Direct patients to the appropriate level of careâwhether scheduling an urgent clinic visit, advising on home care, or routing to the emergency roomâto ensure patient safety and optimize clinic flow.

• Orchestrate cross-functional communication by serving as the central hub between patients, providers, and specialty teams for fast, well-coordinated care decisions.

• Execute and coordinate immediate clinical orders with precision, ensuring acute patient needs are addressed swiftly before or during their clinic visit.

• Empower patients with clear education regarding symptom management and next steps, ensuring they feel supported and safe during transitions of care.

What minimum requirements you’ll need

Licensure / Certification / Registration

• Required Credential(s):

• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date. Valid state license required specific to the state in which the associate works.

• One or more of the following:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.

• Instructor specializing in Basic Life Support obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.

Education

• Diploma from an accredited school/college of nursing required. OR Required professional licensure at time of hire.
