Ecumen is seeking a Home Care Registered Nurse Case Manager (RN) for a Full-Time .8 FTE position servicing Duluth and surrounding areas. The role covers 8am-4:30pm with a structured on-call rotation, weekends and holidays as scheduled.

You’ll provide 1:1 client care, assessments, and care coordination in home and community settings. The ideal candidate has RN licensure in Minnesota, strong communication skills, and experience in post-acute/home care.

• Local travel required to client homes and assignments.

Responsibilities

• Delivers skilled services efficiently and effectively to meet client’s and family’s needs.

This position supports Duluth, Knife River, Two Harbors, Carlton, Cloquet, Wresnshall, Esko, Saginaw and Hermantown.

The Registered Nurse Case Manager position is responsible for providing direct care and case management for a team of clients in accordance with agency standards and policies. This position is responsible for performing skilled nursing functions in the home and community setting as well as serving the nursing needs of clients, families and groups throughout the lifespan.

Opportunity Details:

• The targeted pay range for this job is $39.00 - $43.00 per hour.
• Full-Time .8 FTE M-F 8am-4:30pm - every 5th weekend rotation - Holiday and on-call rotation

2. Completes an initial, comprehensive, and ongoing assessment of clients and family to determine needs. Provides a complete physical assessment and history of current and previous illness.

3. Collaborates with physicians, other health care professionals, clients, and families in developing a comprehensive coordinated plan of care.

4. Develops plan of care which includes objective goals for client improvement in health and functional status. Evaluates plan of care on continuous basis and updates physicians of changes in client status and need for revision of plan of care.

5. Ensures that physician orders are communicated to the agency’s staff with accuracy and thoroughness and understanding.

6. Initiates case conferences to discuss multidisciplinary team responsibilities, client progress, plans for continued care, new problems, etc.

7. Promotes personal safety and a safe environment for clients and co-workers.

Driving Requirements: Local Travel Required

Minimum Required Qualifications:

• 1 year of nursing experience in post-acute care setting

• Must be licensed as a Registered Nurse within the state in which working and must maintain licensure.

• Proficient with Microsoft Office Suite

• Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident’s Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations
