Responsibilities

  • Delivers skilled nursing services in the home and community setting.
  • Performs initial and ongoing patient and family assessments.
  • Collaborates with physicians and care team to develop a care plan.
  • Creates and updates a comprehensive plan of care.
  • Ensures accurate communication of physician orders to the care team.

Description

Hospice RN Case Manager Part Time 48hrs bi-weekly - approximately every 4th weekend on-call rotation and 1-2 weekday on-call shifts per week.

The Registered Nurse Case Manager position is responsible for providing direct care and case management for a team of patients in accordance with agency standards and policies. This position is responsible for performing skilled nursing functions in the home and community setting as well as serving the nursing needs of patients and families.

Schedule:

Part-Time 48hrs bi-weekly Days 8am-4:30pm - On-call rotation - every 4th weekend and Holiday rotation.

Pay:

The targeted pay range for this job is $39.00 - $45.00 per hour. Ecumen considers a variety of factors when determining new hire pay including experience, qualifications, and, when applicable, certifications, licenses, and education.

Essential Job Responsibilities:
  • Delivers skilled services efficiently and effectively to meet patient's and family's needs.
  • Completes an initial, comprehensive, and ongoing assessment of clients and family to determine needs. Provides a complete physical assessment and history of current and previous illness.
  • Collaborates with physicians, other health care professionals, patients, and families in developing a comprehensive coordinated plan of care.
  • Develops plan of care which includes objective goals for patient's end of life care. Evaluates plan of care on continuous basis and updates physicians of changes in patient status and need for revision of plan of care.
  • Ensures that physician orders are communicated to all members of the care team with accuracy and thoroughness and understanding.
  • Initiates case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
  • Promotes personal safety and a safe environment for clients and co-workers.
Driving Requirements:

Minimum Required Qualifications:
  • Associate's Degree in Nursing
  • Must be licensed as a Registered Nurse within the state in which working and must maintain licensure.
  • Ability to communicate effectively both verbal and written
  • Proficient with Microsoft Office Suite
  • Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations
Preferred Qualifications:
  • Bachelor's degree in Nursing
  • Previous experience working with seniors
  • 1 year of nursing experience in post-acute care setting
