Ecumen is seeking a Hospice RN Case Manager (Part-Time) in Duluth, MN to provide skilled nursing and case management in home and community settings. The role supports patient- and family-centered end-of-life care with autonomy in daily scheduling and regular on-call rotations.
Requirements include an Associate's Degree in Nursing and RN licensure in the practicing state, plus strong communication skills and MS Office proficiency.
Hospice RN Case Manager Part Time 48hrs bi-weekly - approximately every 4th weekend on-call rotation and 1-2 weekday on-call shifts per week.

The Registered Nurse Case Manager position is responsible for providing direct care and case management for a team of patients in accordance with agency standards and policies. This position is responsible for performing skilled nursing functions in the home and community setting as well as serving the nursing needs of patients and families.

The targeted pay range for this job is $39.00 - $45.00 per hour. Ecumen considers a variety of factors when determining new hire pay including experience, qualifications, and, when applicable, certifications, licenses, and education.
