Ecumen Hospice Twin Cities in Shoreview, MN is seeking a dedicated, full-time RN Case Manager to provide direct patient care and comprehensive case management for a team of individuals in home and community settings, aligned with agency standards.

Schedule: 80 hours bi-weekly, M-F 8:00 am–4:30 pm, with occasional on-call, one weekend per month and three holidays per year; pay range $40.00–$45.00 per hour; comprehensive benefits included.

Qualifications

  • Must be licensed as a Registered Nurse in Minnesota.
  • Strong verbal and written communication skills.
  • Proficient with Microsoft Office Suite.

Responsibilities

  • Delivers skilled services to meet patient and family needs.
  • Completes initial and ongoing assessments of clients and families.
  • Collaborates with physicians and other health care professionals to develop a plan of care.
  • Develops and evaluates plan of care with objective goals for end-of-life care.
  • Ensures physician orders are communicated to the care team accurately.
  • Initiates case conferences for multidisciplinary team coordination.

Description

Hospice RN Case Manager - Full-Time 80hrs bi-weekly - M-F 8am-4:30pm - Occasional on-call rotation - 3 Holidays per year and 1 weekend per month.

The Registered Nurse Case Manager position is responsible for providing direct care and case management for a team of patients in accordance with agency standards and policies. This position is responsible for performing skilled nursing functions in the home and community setting as well as serving the nursing needs of patients and families.

Ecumen Hospice Twin Cities is hiring a Hospice RN Case Manager. The Ecumen Hospice Twin Cities team focuses on providing life honoring care for patients. Schedule: Full-Time 80hrs bi-weekly M-F 8am-4:30pm occasional on-call requirements - 1 weekend per month and 3 holidays per year.

Pay: The targeted pay range for this job is $40.00-$45.00 per hour. Ecumen considers a variety of factors when determining new hire pay including experience, qualifications, and, when applicable, certifications, licenses, and education.

1. Delivers skilled services efficiently and effectively to meet patient's and family's needs.

2. Completes an initial, comprehensive, and ongoing assessment of clients and family to determine needs. Provides a complete physical assessment and history of current and previous illness.

3. Collaborates with physicians, other health care professionals, patients, and families in developing a comprehensive coordinated plan of care.

4. Develops plan of care which includes objective goals for patient's end of life care. Evaluates plan of care on continuous basis and updates physicians of changes in patient status and need for revision of plan of care.

5. Ensures that physician orders are communicated to all members of the care team with accuracy and thoroughness and understanding.

6. Initiates case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.

7. Promotes personal safety and a safe environment for clients and co-workers.

Driving Requirements: Local Travel Required

Minimum Required Qualifications:

Must be licensed as a Registered Nurse within the state in which working and must maintain licensure.

Ability to communicate effectively both verbal and written

Proficient with Microsoft Office Suite

Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations

Preferred Qualifications:

Bachelor's degree in Nursing

Previous experience working with seniors

1 year of nursing experience in post-acute care setting

Travel Required

Yes . Travel required through the Twin Cities area to visit patients.
