Job summary

The Care Team Hospice is seeking a compassionate RN Case Manager to coordinate patient care in the Highland Heights/Willoughby area. This role emphasizes building relationships with patients and families while ensuring comfort, dignity, and quality of life.

You will collaborate with physicians and the interdisciplinary team to develop and manage individualized plans of care and provide education and support across the care continuum.

Qualifications

• RN licensure in good standing and hospice experience preferred.
• Ability to coordinate complex care plans across disciplines.
• Strong communication and compassionate patient/family support.
