The Care Team Hospice seeks an experienced Hospice RN Case Manager in Springfield, IL to coordinate patient care and implement individualized plans of care. You will collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life at end of life.

Qualifications

• Active RN license required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management per hospice standards.
• Complete visits, admissions, and clinical interventions with timely documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations.
• Collaborate with physicians, caregivers, and interdisciplinary team to coordinate care.
• Educate patients, families, and caregivers on symptom management and end-of-life care.

• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care while providing compassionate support.
