Job summary

The Care Team Hospice is seeking a compassionate RN Case Manager to coordinate patient care, develop plans of care, and work with physicians, caregivers, and the interdisciplinary team to ensure comfort and quality of life for end-of-life patients in the Springfield area.

This role offers Monday–Friday hours, on-call opportunities with compensation, and a supportive environment that values clinical excellence and compassionate care. Active RN license and hospice experience preferred.

Qualifications

• Active RN License required.
• Hospice nursing experience preferred.
• Experience with Homecare Homebase (HCHB) preferred.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards and individualized plans of care.
• Complete scheduled visits, admissions, and clinical interventions with accurate documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management, disease progression, medications, and end-of-life care.

Schedule & Work-Life Balance:

• Monday-Friday, 8:00 AM - 4:30 PM
• On-call opportunities available with additional compensation
• Some locations offer dedicated on-call teams, resulting in backup coverage only
