Job summary

The Care Team Hospice in Lansing, MI seeks a compassionate Hospice RN Case Manager to coordinate end-of-life care and develop individualized plans of care. You will collaborate with physicians, caregivers, and the interdisciplinary team to ensure comfort, dignity, and quality of life wherever patients call home.

Active RN license is required, hospice experience preferred, with strong assessment, communication, and care coordination skills.

Responsibilities

• Provide skilled nursing care, assessments, and symptom management in accordance with hospice standards and physician orders.
• Complete scheduled visits, admissions, and clinical interventions while maintaining accurate documentation in Homecare Homebase (HCHB).
• Respond promptly to patient, family, and caregiver needs, including after-hours and on-call situations, to ensure exceptional patient-centered care.
• Collaborate with physicians, caregivers, and interdisciplinary team members to coordinate care and support positive patient outcomes.
• Educate patients, families, and caregivers on symptom management and end-of-life care with compassionate support.
