Adoration Health is seeking a compassionate Registered Nurse Case Manager to join our hospice team in Prattville, AL. You will provide patient-centered care, assess needs, develop care plans, and collaborate with a multidisciplinary team to support families during a difficult time.

The role emphasizes comprehensive symptom management, medication administration, documentation accuracy, and on-call participation in a supportive, patient-focused environment.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient's wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
