Job summary

Adoration Health is seeking a Registered Nurse Case Manager (Hospice) in Prattville, AL to provide patient-centered hospice care. You will assess needs, develop care plans, administer medications, and support families within a compassionate team environment.

The role emphasizes independent patient management with strong collaboration, adherence to protocols, and on-call coverage as needed. A current nursing license and valid driver�s license are required.

Qualifications

• Graduate of an accredited nursing school with current state licensure to practice in the state of operation.
• One year nursing experience required; three or more years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a vehicle insured as required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patient2s wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient2s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
