Adoration Health is seeking a dedicated Registered Nurse Case Manager for hospice in Prattville, AL. You will provide patient-centered care, develop care plans, and collaborate with an interdisciplinary team to support patients and families during a difficult time.

The role requires nursing licensure, at least 1 year of nursing experience (3+ preferred), and a driver’s license. On-call rotation and office-based in Prattville are included, with benefits typical of hospice services.

Qualifications

• Graduate of an accredited nursing program with state licensure.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Must be a licensed driver with a vehicle insured as per state requirements.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient's family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications
