Hospice of North Alabama is seeking a dedicated Registered Nurse Case Manager in Florence, AL. The role focuses on delivering patient-centered hospice care, assessing needs, and collaborating with an interdisciplinary team to create and implement individualized care plans.

You will provide direct nursing care, manage pain and symptoms, educate families, and participate in on-call rotations. This position emphasizes clinical excellence and compassionate support in a home- or clinic-based setting.

Formación

• Graduate of an accredited nursing program with active RN licensure in the operating state.
• One year of nursing experience required, three+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care and patient-centered practices.
• Commitment to clinical excellence and thorough documentation.

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support patients' families and caregivers.

• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

As a RN Case Manager You will:

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation

• One year nursing experience required, 3+ years preferred

• Experience in hospice or a similar setting preferred

• Knowledge of the hospice philosophy of care

• Commitment to clinical and documentation excellence

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

• Possess and maintain current CPR certification in states where required.
